Health Office Information
The Kenton school district provides an on-site, part-time nurse to students of Mount St. Mary Academy. Joanne Cascio, RN, is available to all students on school days between 8 a.m. - 2 p.m.
Physical Requirements
New York State Education Law and Kenmore-Town of Tonawanda Schools require physical examinations of students in Kindergarten, grades 2, 4, 7 and 10, and all students new to Ken-Ton Schools regardless of grade level.
For a full listing of screening requirements, please click here.
Physicals for all students in special education classes and resource room will be requested as required or deemed necessary.
Physical assessments may be given by the school physician or nurse practitioner. This assessment will include checks of blood pressure, curvature of the spine, and abdominal hernia in girls. Free exams are held in October.
You may choose to have your daughter's exam completed by your own physician. You may download the Mount St. Mary Physical Appraisal form for your doctor to complete or obtain one from the Health Office. The required physical for the 2011-12 school year must be dated after 09/2011.
Sports Team Requirements
To play a sport at Mount St. Mary Academy, you must:
- Have a valid physical (dated within one year the sport begins) on file in the Health Office;
- Return to the nurse the athletic sport consent form, completed and signed by a parent or guardian. Once the student has a valid physical/completed athletic sport consent form, the nurse will sign the sport consent form and the student must give this form to the coach (or the nurse will forward the form).
*For Fall Sports, if consents are mailed in (by August 1st), the nurse will forward the necessary consent form to the coach only if the student states a fall sport on the form. If there are any changes in your daughter's health between August 1st and the try out date, please notify the nurse. The nurse's signature must be on the consent form in order to try out.
